Please submit the Daycare/Sitter Form asap if your HES student is going to/from a Daycare/Sitter this school year!
This form must be submitted each year - instructions from previous school years will not be followed automatically. A new form must be filled out for 2024-2025 even if the information is the same as this year. If the form is not filled out, the Transportation Dept. will be delayed in assigning your child a bus and it will be your responsibility to drive them to their Daycare/Aftercare until a bus is assigned.
DO NOT COMPLETE THIS FORM IF YOUR CHILD TAKES THE BUS FROM YOUR HOME IN THE MORNING AND TO YOUR HOME AT DISMISSAL

Forms must be submitted by July 26 to be in effect for the first day of school.
Forms submitted between July 27 and September 5, may take up to two weeks for processing which means you will be responsible for transporting your child to/from daycare during that time.
Forms submitted after September 6, may not be processed until September 29.
THIS FORM MUST BE SUBMITTED EACH YEAR.
A SEPARATE FORM IS REQUIRED FOR EACH CHILD.
Important! If you have a child entering kindergarten, do not complete this form until you know if your child has AM or PM kindergarten.
